Commitment Club

By entering into this Agreement and signing up for the Define Beauty Medspa Commitment Club, you agree to purchase a facial from the listed choices included, on every Membership Charge Date.

 

In addition to your monthly Facial, you’ll receive the following benefits:

• 10 -15% off skincare purchases at all times

• 10 -15% add - ons and additional services

• 1 Complimentary add-on per quarter

• 10 -15% off Advanced Treatments including Microneedling or IPL Brightening & Toning Treatment

  • Cannot be combined with other promotions or offers

Monthly Payment

Your card will be automatically charged on the same day of each month. Gratuity is not included in monthly payment.

If payment is declined, your Commitment Club will automatically freeze until payment is collected. 
If not resolved within 30 days of payment due date, Define Beauty Medspa may retain expenses incurred or the portion of the total price of this Agreement representing the services you used or completed, and provided further, that Define Beauty Medspa may demand the reasonable cost of goods and services which you consumed or wish to retain after cancellation.

Upgrades
If you decide you would like to upgrade or change your Commitment Club to another level, a new signed contract needs to be completed before the next billing cycle. If you would like to add an upgraded service to your monthly facial treatment such as adding a chemical peel, mask, or another area, please discuss the discounted pricing including in the club with your Aesthetician. 

Discounts/Sharing

Cannot be combined with any other offers, gift certificates, or promotions. Commitment Club benefits cannot be shared with any other patient. 

Minimum Commitment / Cancellation
We require 90 days notice for cancellation. If you wish to cancel, please let your Aesthetic provider know and we will have you complete a signed cancellation notice.  Memberships will be canceled effective 90 days following our receipt of such cancellation requests.

REVOCATION OF MEMBERSHIP:

Define Beauty Medspa reserves the right to revoke and cancel this Membership at any time for any reason, in which case you will, no later than fifteen (15) days after such cancellation, receive a refund of all monies paid pursuant to this Agreement, provided that Define Beauty Medspa may retain expenses incurred or the portion of the total price of this Agreement representing the services you used or completed, and provided further, that Define Beauty Medspa may demand the reasonable cost of goods and services which you consumed or wish to retain after cancellation.
